Standard 4: Comprehensively conducts assessments RNs accurately conduct comprehensive and systematic assessments. They analyse information and data and communicate outcomes as the basis for practice.

Objective: To demonstrate the fundamental ability to do assessment like vital signs, history taking, BGL monitoring, and cephalocaudal (head to toe) assessment.
Rationale: Obtaining the basic indicators of the patients health status would determine what kind of care and treatment they require and the urgency for their treatment regimen.
Strategy: To practice more on vital signs taking like BP monitoring, measuring the oxygen saturation of the body with the use of pulse oximeter, manual reading of Pulse rate and Breath cycles per minute.
Evidence: Attended clinical laboratory topic2 on vital signs taking. Finished LT lessons provided by the unit.
